Please read the following terms carefully before submitting your application.

1. Program Eligibility

The Residential Internet Program is available exclusively to families with children currently enrolled in Cleveland Metropolitan School District (CMSD). Applicants must provide valid CMSD Student IDs for all students listed on this application. Providing false or inaccurate information may result in disqualification and removal from the program.

2. Service Provision

Internet service is provided to qualifying households at no cost as long as at least one student listed on the application remains actively enrolled in CMSD. Service availability is subject to infrastructure limitations and program capacity. CMSD and DigitalC reserve the right to modify, suspend, or terminate service with reasonable advance notice.

3. Acceptable Use

The internet service provided through this program must be used for lawful purposes only. Prohibited activities include, but are not limited to: illegal downloads, distribution of harmful content, unauthorized network access, or any activity that violates federal, state, or local law. Misuse of the service may result in immediate termination without notice.

4. Privacy & Data Collection

By submitting this application, you consent to the collection and use of your household and student information for the purposes of administering the Residential Internet Program. Your information will not be sold to third parties. It may be shared with CMSD and authorized program administrators solely for service delivery and eligibility verification purposes. All data handling is governed by applicable privacy laws including FERPA.

5. Annual Re-enrollment

Participation in the program is subject to annual review. Families may be required to re-verify enrollment status at the beginning of each academic year. Failure to re-enroll may result in suspension of service.

6. Equipment & Installation

Any equipment provided as part of this program remains the property of the service provider. Equipment must be returned upon request or upon termination of service. Damage to or loss of program equipment may result in fees assessed to the household.

7. Accuracy of Information

You certify that all information provided in this application is true, accurate, and complete to the best of your knowledge. Submitting false information is grounds for immediate removal from the program and may be subject to further action.

8. Modifications to Terms

CMSD and DigitalC reserve the right to update these terms at any time. Continued use of the service following notification of changes constitutes acceptance of the revised terms.
